By law, the credit bureau has 30 days to respond to a dispute and notify you of the results. So the notation it's under dispute must appear prior to that.
From my own experience, factoring in the time I mailed the dispute letter, it takes a couple of weeks. They seem to place the notation on the record at the time the are processing your request.
Results or changes will depend on how long the creditor takes to respond back to the bureau. Again, by law they must do so in 30 days, or the bureau must remove the report from your records.

My husband had to dispute some items on his credit report, and the changes were done fast -- in less than 6 months. Some of his questionable black marks just disappeared from his credit report. You have to give a strong explanation for disputing the item in the credit report

The item will be marked "consumer disputes - reinvestigation in process" as soon as the dispute is entered in their system. However, I'm not sure why you are asking the question since the dispute is only a remark and doesn't change your credit score until the investigation is complete(approx. 30 days) and the information is verified/updated/deleted.

Maybe never. If you dispute an item, the credit bureau has to ask the creditor to verify the information. If the creditor says, "Oops, our bad," it will be corrected. If the creditor says it's correct, there will be no change. The bureau has to notify you of the response, and if it is not going to be corrected, you can ask to have your side of the story, stated briefly, sent with any credit report sent out after that.
